REGISTRATION

White County Memorial Hospital’s Patient Access Services division is committed to providing excellent customer service.  We offer comfortable surroundings and strive to deliver courteous and compassionate care.  Our patients always come first at White County Memorial Hospital.  

The main hospital telephone number is (574)583-7111.  A receptionist is readily available in person and/or by phone to greet and direct patients and visitors to their appropriate destinations.  Language lines are available for interpretation.  Our Main Registration department features four private handicap- accessible registration rooms for admitting inpatients and outpatients.  We respect the trust you place in us and vow to keep your personal and medical information confidential.

Upon registration, we require a copy of your insurance card if applicable, driver’s license and/or a photo identification card.  Payment of deductible and co-pays are expected at the point of registration. 

The Main Entrance is open from 7:00 a.m. to 7:00 p.m., seven days a week for your convenience while visiting patients.  Our Main Registration office hours are 7:00 a.m. to 5:00 p.m., Monday-Friday.   After hour registrations should report to the Emergency Registration desk through the Emergency Entrance.  Our Emergency Department is available 24 hours a day, seven days a week.   

 

PATIENT FINANCIAL SERVICES

Insurance and Professional Billing
As a courtesy to our patients, White County Memorial Hospital submits bills on your behalf to your insurance carrier.  Our Patient Financial Services division offers many years of billing experience and is qualified to submit claims to Medicare, Medicaid, Blue Cross, and all third party insurance carriers.  We provide your insurance company the required documents needed to process your claim; however, it may become necessary for you to contact your insurance company to supply additional information or to expedite payment.  Please note that your policy is a contract between you and your insurance company, and you have the final responsibility for payment of your hospital bill.

Any Hospital expenses incurred as the result of a vehicle accident is considered the personal responsibility of the patient.  Your bill will be sent directly to you to forward to the appropriate party; however, you are ultimately responsible for payment.  Attorney letters of protection are not honored by the Hospital. 

 

The Patient Financial Services office hours are 8:00 a.m. to 4:00 p.m. Monday-Friday.  If you have questions regarding your insurance claim, please contact a Patient Financial Service Representative at one of the following numbers:

You may receive bills from physicians that you did not see in person for specific tests or treatments.  These bills are for professional services provided by these doctors in diagnosing and interpreting test results while you were a patient.  Pathologists, radiologists, cardiologists, anesthesiologists, and other specialists perform these services and are legally required to submit separate bills.  In addition, when you receive emergency services, you will receive a separate bill from Lafayette Emergency Physicians for the physician service you received.  Should you have any questions about these bills, please contact the number listed on the statement you received from them.

 

CREDIT AND COLLECTION SERVICES

Estimates, Deposits, and Refunds
Our staff is trained to prepare estimates for you in order to satisfy your financial responsibility. These are only estimates and may not represent the full scope of charges you actually incur. If the deposit turns out being more than your final bill, you will be refunded once the insurance balance has been settled. If there was no insurance involved, we will refund your money once all charges have been totaled provided an overpayment exists.    

Non-Insured Patient and Services Not Covered By Insurance
All balances are due upon receipt.  If your claim is to be billed to an insurance company, please contact our office immediately with the insurance information.  To offset any adverse collection efforts, prompt payment for service is expected.

Financial Hardship Assistance Program
White County Memorial Hospital promotes the health and well-being of the community it serves; therefore, we offer a charity care program to individuals with limited financial resources who meet the established criteria.  Eligibility criteria are based upon Federal Poverty Guidelines and are updated annually in conjunction with the published updates by the United States Health and Human Services.  A redetermination is required for each new visit. 

To be considered for financial assistance, the patient must cooperate with the facility by providing the required information and documentation in order to determine eligibility for financial assistance.    

Our Credit services hours are available 8:00 a.m. to 4:00 p.m. Monday, Thursday, & Friday, in addition to Tuesday 8:00 a.m. to 7:00 p.m. and Wednesday 8:00 a.m. to 12:30 p.m. You may contact our Financial Counselor at (574)583-1772.
